- Once you have been directed to the survey page scroll down to the login box:

- Once you login the following screen will appear:
For your ID please use: first initial, last name - S (place an S in the section next to your name so that we can determine when the survey was taken.
Complete the remaining information. Make sure to indicate the Spring (January -June) Semester.
- Click the Next button.
- This will take you to the survey to complete.
School Technology Survey
Click on the banner above to access the 2006-2007 Louisiana School Survey and Evaluation Report. If you do not have your school's username and password email Tammy Seneca.
WBR Technology Survey
- Go to the WBR Moodle Website.
- You will see the following screen:
Click on the first "course" - EOY Technology Survey.
- You will be prompted for a username and password:
Your username is teacher and the password is techsurvey.
- Once you login you will see:
The only option you will have is the EOY Technology Survey that you see in blue. Click on the blue words in order to access the 15-question survey.
- You will see the following screen:
Click on Attempt quiz now or Continue the last attempt to answer the 15 questions!
- Answer the questions then submit your quiz! Clicking submit on each question. Don't worry about incorrect answers... it is just the setup of the program. There are no right or wrong answers!
- Click Submit All and Finish. You are done!! THANKS!!
